California Community Transitions (CCT)

Care Coordination & Transitional Care Services

Star Nursing is leading the way for Medi-Cal members who are currently in a skilled nursing facility and would like to transition back into community living (home, apartment, or residential care facility). Visit the state website for more information about this program here.

Star Nursing is an approved Lead Organization for the CCT program. Our Transition Coordinator will explore living choices and explain available services, such as home and vehicle adaptation, assistive devices, transportation options, wheelchairs, home modifications, such as grab bars and ramps and other medical equipment, self-care training, and the need for a potential caregiver.

How to qualify for CCT Program

You must have Medi-Cal or be Medi-Cal eligible You have resided in a medical facility like a hospital or skilled nursing facility for over 60 days You require services such as those you are currently receiving in the facility to live independently.

Star Nursing’s Care Team will be with you every step of the way.

Our Care Team will provide Case Management services for 1 year from the time you move to your own place.

Program Support

Star Nursing’s Transition Coordinator helps assist with your recovery by assisting with relocating to independent living by:
  • Coordinating and locating affordable housing
  • Home set-up
  • Adaptive equipment
  • Transportation options
  • Self-care training
  • Research, interview, and hiring of a care provider, if needed
  • Creating an overall plan to return to community-living safely
