Enhanced Care Management (ECM)
We are transforming healthcare for high-need Medi-Cal members by delivering comprehensive care that extends beyond the hospital or doctor’s office.
ECM is a new statewide Medi-Cal benefit available to eligible members with complex needs, including:
- Access to a single Lead Care Manager who provides comprehensive care management and coordinates their health and health-related care and services.
- Connections to the quality care they need, no matter where members seek care—at the doctor, the dentist, with a social worker, or at a community center.
Star Nursing is proud to be an official provider of Enhanced Care Management (ECM) services in California. Our program is designed with a personalized approach, embracing multilingual proficiency, cultural understanding, and an unwavering dedication to supporting our members and communities. Each member is paired with a compassionate and experienced Care Coordinator who provides consistent support throughout their healthcare journey.
Our care coordinators are backed by a collaborative team of professionals, including skilled nurses, housing navigators, continuity of care specialists, transitional care specialists, resource specialists, street outreach experts, and ECM leaders. Together, we ensure our members receive the highest level of care, fostering health and well-being at every step.
How it works
Care Coordinators can assist members with filling out paperwork, help members coordinate with their doctors, assist members set up free transportation to and from their doctor visits, help establish income/find a job, assist with creating and building resume and more.
- Individuals Experiencing Homelessness
- Individuals At Risk for Avoidable Hospital or ED Utilization (Formerly “High Utilizers”)
- Individuals with Serious Mental Health and/or SUD Needs
- Individuals Transitioning from Incarceration
- Adults Living in the Community and At Risk for LTC Institutionalization
- Adult Nursing Facility Residents Transitioning to the Community
- Children and Youth Enrolled in California Children’s Services (CCS) or CCS, Whole Child Model (WCM) with Additional Needs Beyond the CCS Condition
- Children and Youth Involved in Child Welfare
